If Game of Thrones represents a pseudo-medieval time, there is probably less exploitation of women than actually occurred in those days, and the women probably have far more power than they really did. In other words, it's probably a sugar-coated version of those times.
On the other hand, it is actually supposed to be a fantasy universe completely divorced from human experience, then there is exactly as much sex and violence as its creators imagine in that universe.
I see no possible case to argue for less sex and violence, although I could see why socially conservative people would feel that the sex and violence should be portrayed more subtly. Unfortunately, those people don't seem to understand why other people pay a premium to subscribe to HBO in the first place, which is to get away from the expurgated and saccharine worlds portrayed by broadcast TV and basic cable.
