Fake in what sense? If you mean that a real picture of Helen Mirren has been superimposed upon a real picture of a VHS box from the movie, you're definitely correct.
That might be. I never suspected it because Helen does more explicit nudity than that in the film, and her body looks pretty much the same as that (http://www.fakes.net/mirren_helen157.jpg), so it wouldn't make sense to use a fake.
